Researchers calculated things like employee sick days, extra gasoline cost (a reality of obesity), and a loss of productivity to find the annual cost of obesity. It tallies up to almost ,000 per year for men, and almost ,000 for women. The numbers were substantially less for those classified as overweight. In regard to the gender inconsistency, other studies have shown heavier women earn less than thin women, while wages remain fairly consistent throughout the varying male weight designations. Bariatric surgery in Michigan is a possible solution to the weight problem, but sadly sexism in the workplace is an entirely different problem, with a much more complex solution, if there is one at all.

Researchers assumed wages would take a hit across the board for those suffering from obesity, but were surprised to find women were at a greater disadvantage. Heath economist Eric Finkelstein at the RTI Institute worked extensively with the CDC to determine their health costs of obesity. He says he is concerned the incentives for employers to combat the epidemic internally just aren’t there. It seems the short duration employees work for companies (an average of 4.5 years) mask the progressive nature of obesity-related health conditions. An employer would have to keep an employee for at least 20 years to reap the benefits, and eliminate those extra costs, of helping his or her employees achieve and maintain a healthy weight.

Variability in blood pressure appears to predict the risk of stroke more powerfully than high average or usual blood pressure. Three papers published simultaneously in The Lancet, and another study in The Lancet Neurology, all support the idea that variability is the more powerful indicator.

These studies by Professor Peter Rothwell and colleagues at the University of Oxford have major implications for diagnosing, and treating high blood pressure to prevent heart disease and stroke.

Prior to these studies, it was believed that the underlying average blood pressure determined most of the risk of complications from hypertension (raised blood pressure), and all of the benefit from the drugs that are used to lower blood pressure.

Improved Direction in Medical Guidelines Means Better Healthcare

In these studies, Rothwell’s team found that variability of blood pressure (BP) between visits to clinics was a strong predictor of angina, heart failure, myocardial infarction, and stroke, independent of mean BP.

This contradicts assumptions in current medical guidelines that patients with only occasional high readings do not require treatment. Now, it is becoming clear that such patients have a high risk of stroke and other complications. Occasional high BP that may be called episodic hypertension are associated with a high risk of stroke, and can no longer be considered to be benign.

Rothwell’s team also analyzed previous trials comparing the effects of blood-pressure-lowering medications. They found that different blood-pressure-lowering drugs have different effects on BP variability.

Certain prescription drugs increase this variability, which is dangerous, and others decrease variability, which is helpful. The researchers also found that these effects correlated with differences in the effectiveness of the drugs in preventing stroke.

Eating plenty of fruits and vegetables, exercising regularly, and keeping your weight under control can lower systolic BP by about 10 to 15 mmHG. In the early 1800s, Narragansett Pacers and Canadian Pacers were crossbred by breeders in the U.S. state of Tennessee while looking for a horse that could handle the hilly terrain of the area. Confederate Pacers and Union Trotters were added to the gene pool during the Civil War, creating the sturdy Southern Plantation Horse, which was also known as the Tennessee Pacer. Thoroughbred, Standardbred, Morgan, and American Saddlebreds were later added to refine and add stamina to this gaited horse. In 1885, a colt named Black Allen out of the Hambletonian family of Standardbreds and a Morgan mare was born. He became the foundation sire of the Tennessee Walking Horse, also known as the Tennessee Walker, or TWH.

The Tennessee Walking Horse Breeders’ and Exhibitors’ Association (TWHBEA) is the official registry and was founded in 1935. Headquartered in Tennessee, it is the oldest organization devoted to the Tennessee Walking Horse. In order to maintain breed purity, the stud book was closed in 1947 and now every TWH born after that date must have both parents registered with TWHBEA in order to be registered themselves. There are more than 430,000 registered Tennessee Walkers throughout the world and the breed has become the second-fastest growing breed in the nation.

Every year, a ten-day exhibition in the town of Shelbyville, Tennessee called the “Tennessee Walking Horse National Celebration” draws over 30,000 breeders, exhibitors, and spectators. The show is so large that Shelbyville bills itself as the “Walking Horse Capital of the World,” and now the Tennessee Walker is the official state horse of Tennessee.

The Tennessee Walker is a tall horse ranging from 13.2 to 18 hands with an average of 15-17 hands high and a weight of 900 to 1200 pounds. It has a long neck, a long sloping shoulder, a long sloping hip, a fairly short back and an elongated stride. The head is traditionally large but refined and has small well-placed ears. At least 20 color choices are available. Black, roan, chestnut sorrel, bay, champagne and pinto are the most common. The TWHBEA now recognizes the sabino pattern since many past registered roans were actually sabinos.

A TWH foal is never gray at birth but may begin to turn gray when the foal coat is shed or maybe not until several years later. When they start to turn gray, it is progressive and they continue to lighten until they are nearly white. If they are white with specks of color, they are referred to as a flea bitten gray. The birth color remains on the registration if the foal turns gray, but gray is then added to the base color, i.e., black/gray, palomino/gray, etc.

The Tennessee Walker has three distinct gaits: the flat foot walk, the running walk or “big lick”, and the rocking horse canter, all of which the breed is famous for, with the running walk being an inherited, natural gait unique to this breed. Many are also able to perform the rack, stepping pace, fox-trot, single-foot and other variations of the famous running walk.

The Flat Walk is a brisk, 4 to 8 mile an hour, long-reaching four-cornered gait where each foot hits the ground separately at regular intervals. The action of the back foot slipping over the front foot’s track is known as overstride and is unique to this breed. A Tennessee Walking Horse will nod its head and swing its ears in rhythm with the cadence of its feet. This distinctive nodding head motion, along with overstride, are two features that are unique to the Tennessee Walking Horse.

The Running Walk is the extra-smooth, gliding gait for which the walking horse is most noted and is referred to as “big lick”. It is the same as the flat walk only at 10-20 miles per hour. As the speed increases, the horse overstrides the front track with the back by between 6 to 18 inches. The running walk has a motion that gives the rider a feeling of gliding through the air and the Tennessee Walker will continue to nod and swing its ears during this gait. Some horses even click their teeth during the big lick.

The Canter is much the same as other breeds, but the Tennessee Walker seems to do it in a more relaxed way. This gait gives the rider a feeling of ease with lots of spring and rhythm. In fact, the canter lifts the front of the body, giving an easy rise and fall motion that gives it the “rocking-chair” name.

The TWH easily adapts to both English and Western tack and attire and there are two main categories of competition under both disciplines: Performance and Flat Shod.

The Flat Shod variety has grown tremendously popular due to the easy training of the breed without the need of a professional trainer, and the naturally inherited gaits with no foot enhancements, although the plantation class may wear a heavier shoe. If used for pleasure riding, no special shoes are required, although when they are shod, it will be at a slightly lower angle with more natural toe than some of the other western type horses. A record of these angles and foot lengths should be kept handy for when the farrier comes by.

The Performance Horse (sometimes referred to as padded or built up) is shown in English attire and tack. It executes the basic gaits with flashier and more animated movements, particularly the “big lick.” They appear to sit on their hind quarters, lifting their front feet high off the ground. They may also use tail braces.

Additionally, Performance horses are shod in double and triple-nailed tall pads to add dimension to the hoof to provide a sounder base and to change certain angles and paths in the motion of the front hooves. Additionally, lightweight chains, or “action devices” are worn around the fetlocks of the front legs to accentuate the gaits to make them showier. Pads are training devices and an integral part of the training because when utilized properly, they can aid greatly in accentuating the gaits of the show horse. However, their effectiveness will vary with the individual horse.

However, there is a dark side to the Tennessee Walking Horse’s showy gaits. The use of pads and the accompanying chains that go with them is highly controversial, since some people put irritating chemicals on the pasterns to make them sensitive and sore when the chains touch. The practice has become so widespread that USDA inspectors are now attending shows to look for evidence of “soring” which is the illegal practice of deliberately injuring a horse’s front feet to get it to step higher.” Some owners even use eye drops to temporarily blind their horses which makes them lift their hooves higher to try to feel their way around. These horses also tend to have joint problems because we tend ask them to do things they weren’t designed to do. Show horses tend to shift their center of gravity to the rear, thus placing more stress on the hind limbs (especially the hock and pastern joints). The goal is to move with a lot of high front-end action and this is particularly true of the Tennessee Walker in competition. Horses that have more of a weight load on the rear legs are going to be prone to hock, rear fetlock, and stifle injuries and disease.

There are also a couple of genetic concerns with the Tennessee Walking Horse. Equine Polysaccharide Storage Myopathy (EPSSM) which appears in the form of muscular atrophy, abnormal gait, back pain, and post anesthetic myopathy have been described and are thought to be inherited.

Equine Recurrent Uveitis (ERU) or “moon blindness” can lead to blindness if not treated. The University of Minnesota is currently conducting research to determine if there is a genetic factor involved and a potential gene region that may be linked to the condition may have been identified.

While these horses are famous for flashy movement, they are quite popular for simply pleasure riding. Combined with having an easygoing, docile temperament and being easy to train, their naturally smooth and easy gaits insure the popularity of the Tennessee Walker as the “world’s greatest show, trail, and pleasure horse”.

A new scientific research study conducted at the University of Kentucky College of Medicine provides further evidence that people with high blood pressure can find relief through meditation—but the study also found that not all forms of meditation are equally effective. The study compared findings from research on several well-known types of meditation and relaxation practices, and found that the only mind/body practice that produces significant changes in blood pressure is the Transcendental Meditation technique. 

 

According to a definitive new meta-analysis of 107 previous studies on stress reduction programs and high blood pressure, published in the American Journal of Hypertension (Vol. 21, 3: 310-316), the Transcendental Meditation technique was found to produce a statistically significant reduction in high blood pressure—an effect not found with other forms of relaxation, meditation, biofeedback or stress management. 

 

Significant Reductions in Blood Pressure

 

The new meta-analysis reviewed randomized, controlled trials published in peer-reviewed scientific journals over the past decades concerning stress reduction and relaxation methods used by participants with high blood pressure. Blood pressure changes through the Transcendental Meditation technique included average reductions of 5.0 points of systolic and 2.8 points of diastolic blood pressure, changes which were statistically significant, according to the review. These changes associated with Transcendental Meditation practice were consistent with other controlled studies showing reductions in cardiovascular risk factors, improved markers of heart disease, and reduced mortality rates among participants in the Transcendental Meditation program. 

 

Transcendental Meditation “equivalent to adding a second hypertensive agent”

 

“The magnitude of the changes in blood pressure with the Transcendental Meditation technique are at least as great as the changes found with major changes in diet or exercise that doctors often recommend,” said Dr. James Anderson, principal author of the study. “Yet the Transcendental Meditation technique does not require changes in lifestyle. Thus many patients with mild hypertension or prehypertension may be able to avoid the need to take blood pressure medications—all of which have adverse side effects. Individuals with more severe forms of hypertension may be able to reduce the number or dosages of their BP medications under the guidance of their doctor.”  

 

Dr. Anderson stated further: “Adding Transcendental Meditation is about equivalent to adding a second hypertensive agent to one’s current regiment, only safer and less troublesome.” 

 

Reduced Risk of Heart Attack and Stroke

 

Anderson added that long-term changes in blood pressure of this magnitude are associated with at least a 15 percent reduction in rates of heart attack and stroke. “This is important to everyone because cardiovascular disease is the number one cause of death in the U.S. and worldwide,” Anderson said. The study’s biostatistician, Maxwell Rainforth, assistant professor of Physiology and Health Statistics at Maharishi University of Management, said the meta-analysis used state-of-the-art statistical methods to review 107 published studies in the field of stress reduction, relaxation and blood pressure. “The twenty-three separate studies included in the final analysis met well-known criteria for high scientific quality. That is, these studies used repeated blood pressure measurements and participants were randomized to either a stress reduction technique or placebo-type control for at least eight weeks. The data we used are all published in peer-reviewed scientific journals,” Rainforth said. 

 

Side Benefits of TM vs. side effects of drugs

 

According to Dr. Robert Schneider, director of the Institute of Natural Medicine and Prevention and co-author, this rigorously conducted meta-analysis indicates that the Transcendental Meditation technique is distinctively effective compared to other scientifically studied techniques in lowering high blood pressure. “For those 100 million Americans with elevated blood pressure, here is a scientifically documented, yet simple and easy way to lower blood pressure without drugs and harmful side effects. In addition, related studies show an integrated set of positive ‘side benefits,’ such as reduced stress, reduced heart disease levels and longer lifespan with this technique to restore balance in the cardiovascular system, mind and body,” Schneider said. 

 

Overturns a previous study on meditation

 

The new meta-analysis was co-authored by researchers at the University of Kentucky College of Medicine, in Louisville, and at the NIH-funded Institute of Natural Medicine and Prevention at Maharishi University of Management in Iowa. 

 

According to Dr. Anderson, the findings of this new study rebut a July 2007 report sponsored by the Agency for Healthcare Research and Quality and the NIH-National Center for Complementary and Alternative Medicine, which concluded that most research on meditation is low quality and found little evidence that any specific stress reduction effectively lowers blood pressure.  

 

The new meta-analysis identified all high quality meditation studies published through 2006 and rigorously analyzed their results, which the previous government report failed to do. Anderson said the new meta-analysis includes only high quality studies on all available stress reduction interventions. The studies on the Transcendental Meditation technique were conducted at five independent universities and medical institutions, and the majority of them were funded by competitive grants from the National Institutes of Health.

The Colorado Ranger Horse was named for its Colorado High Plains origin. Verbal references to those “range bred” horses eventually led to their being more commonly known as Rangerbreds or Rangerbred Horses. But despite its appearance, the Rangerbred is not a type of Appaloosa even though many Rangerbreds are double-registered with the Appaloosa Horse Clubs of both the United States and Canada. It has its own unique heritage.

Colorado Ranger Horses were bred for being cow savvy, and can anticipate the movements of cattle, and for their performance capabilities. They excel in ranch work with great stamina and do well in endurance competitions.

Colorado Ranger Horses are refined horses due to their Arabian/Barb ancestry and are compact animals, with powerful hindquarters. Like most popular breeds, Rangerbred sizes range from 14.2 to 16+ hands with the average height at 15.2 hands, and they have good dispositions.

Although the breed as we know it today is considered to have originated in America, its roots can be traced back to Constantinople, Turkey.

During 1878, General Ulysses S. Grant visited Sultan Abdul Hamid of Turkey as part of a world tour. The Sultan, in showing his regard for the General, gave him the gift of two desert stallions; a blue-gray Barb named Linden Tree and a gray Siglavy-Gidran Arab named Leopard. These horses are listed in the studbooks of both the Jockey Club and the Arabian Horse Club and their influence has touched almost every breed of horse in the United States.

These two horses went to Virginia at first, where they were used as foundation sires in a new breed of light-harness horse called the Americo Arab. But when the automobile was invented, along with other difficulties, the breeding project was discontinued in 1906 and his herd was disbanded.

So, Leopard and Linden Tree spent a season in Nebraska and sired a few foals, some spotted or colored, from the native mares of the General Colby Ranch. A.C. Whipple, of Kit Carson County in Colorado, obtained a herd of broodmares from the Colby Ranch who were all sired by either Linden Tree or Leopard. In addition, a black-eared white stallion named Tony was used as the herd stallion, because he was double bred to Leopard and was part of the family’s extensive line-breeding program using Tony and his sons.

In the early 20th Century, Mike Ruby, of the Lazy J Bar Ranch, bought one of Tony’s sons, a stallion named Patches and Max, son of Waldron Leopard. He used these stallions as the foundation sires of the new breed, in which unusual coloring was seen more and more frequently in his herd of more than 300 mares.

So, in essence, the Colorado Ranger was developed by Mike Ruby, who kept meticulous records on every foal that he bred. These records included foaling dates, coat patterns and complete pedigrees and are still in existence today with all horses still being recorded by hand in these ledgers, as well as by more modern methods in the Colorado Ranger breed registry.

After two leopard-patterned stallions were displayed at the Denver Stock Show, they created such a sensation that Mike Ruby was urged by the faculty of what is now Colorado State University to name this new breed of horse. And so the Colorado Ranger Horse was officially named in 1934 to reflect that they originated in Colorado and were bred and raised under range conditions.

And with the naming of the breed came the breed registry. The Colorado Ranger Horse Association (CRHA) is an older registry than the Appaloosa Horse Club (ApHC). In fact, it is the oldest of the western horse breed registries still in existence in the United States. It was founded in 1935 by Mike Ruby, who was its first president until his death in 1942. Its corporate charter was granted in 1938. Ironically, its home office is currently in Pennsylvania.

In the beginning, registration was limited only to the first 50 CRHA members, so a lot of true Rangerbreds were not allowed to be registered with CRHA. However, those horses with the appropriate color patterns were gladly accepted by the Appaloosa Horse Club which was another breed registry that was founded several months later. In 1964, the CRHA lifted the fifty member limit and registration was opened to all horses meeting the pedigree requirements, regardless of the owner’s membership status. This enabled the CRHA to register many of the Appaloosas that had Rangerbred heritage that were “lost” to the organization for so many years.

About 90% of all registered Rangerbreds are also registered with the Appaloosa Horse Club, but not all Appaloosas are eligible for registration with the CRHA, unless they have the required pedigree that shows a direct descent from one of the two foundation stallions, Max #2 and/or Patches #1 in an unbroken line. Patches #1 was purchased from the Whipple Ranch and traces to both Leopard and Linden Tree. Max #2 came from the Governor Oliver Shoup ranch at Colorado Springs and is descended from Waldron Leopard.

While many Colorado ranger horses display the same color patterns as the appaloosa, the CRHA is a bloodline registry, not a color registry. In fact, color and markings are not considered in eligibility for registration, only ancestry is. The breed’s founder wisely decided that a horse’s ability has nothing to do with color of his coat.  

As with the Appaloosa Horse Club (ApHC), the CRHA recognizes the same approved outcrosses as the ApHC. The following breeds are considered as acceptable outcrosses for the Rangerbred and may be used in a CRHA Breeding Program: The American Jockey Club (TB), The American Quarter Horse Association (AQHA)., ApHC of USA, Canada & Foreign, The Arabian Horse Club (AHC), ARA-APP, and the International Colored Appaloosa Association (ICAA) (with certain reservations). The outcrossed mare must be registered with one of the above registries. Paints & pintos are not among these approved outcrosses.

Research indicates that one out of every eight Appaloosas is of Rangerbred heritage and also eligible for CRHA registration.

An received degree in one of the provided areas: chiropractic (DCM,DC), medicine (MD), dentistry (DMD,DDS), optometry (OD), pharmacy (Pharm.D), osteopathic medicine (DO), podiatry (DPM, DP, PodD); ministry/divinity (MDiv, BD), rabbinical & Talmudic studies (Rav,MHL), law (JD, LLB); and veterinary medicine (DVM).

6.Certifications. The document certifying you have met a particular standard in academic degree, continuing education, testing and experience. Certifications differ by industry and type. Many professions (like teaching and account) need ongoing certification when many technical-vocational area need certifications based upon a set of major courses (and typically not needing a college degree).

McLaughlin is hoping for that victory during the Blue Raider’s April 24th home game against University of Arkansas Little Rock.

What You Need to Know about Stroke

Is there anything left out there that doctors have decided is not dangerous to eat. Well, yes – dark chocolate as long as you don’t eat too much. Dark chocolate has a surprising number of health benefits. For one, you may be able to lower your blood pressure by eating a few pieces of 70% chocolate.

Dark chocolate is also known as “bittersweet” or “semisweet” chocolate contains a high percentage (up to 75%) of cocoa solids, and little (or no) added sugar. Milk chocolate contains powdered or condensed milk; it is a sweet, mild-flavored type of chocolate. It contains approximately 20 percent cocoa solids. White chocolate is not classified as chocolate because it does not come from cocoa beans. However, it is made from cocoa butter to which milk, sugar, and vanilla extract have been added, and it is similar to chocolate in texture.

A few years ago researchers at Harvard discovered that the flavonoids that are present in cocoa help improve blood flow to the brain. One of the causes of mental decline as you age is decreased blood flow to the brain. As you can see dark chocolate has the highest percentage of cocoa therefore contains the largest concentration of flavonoids. Foods that contain high amounts of flavonoids include blueberries, cranberries, blackberries, red and yellow fruits (especially red grapes) and vegetables and red wine. Black and green teas are also high in flavonoids.

In a study at the University of California, San Francisco though both groups were given dark chocolate for two weeks, only one group received chocolate containing flavonoids. The other group’s chocolate bars contained no flavonoids. Dark chocolate contains a rather vigorous type of flavonoid called epicatechin. Epicatechin keeps cholesterol from accumulating in the blood vessels hence reducing the risk of stroke and heart disease. There is also evidence to support that they reduce the probability of cancer and diabetes.

At the end of the two weeks each group was tested to see how well the blood vessels dilate and relaxed which is considered an indicator of healthy blood vessel function. The test subjects that consumed the dark chocolate containing flavonoids performed better on the blood vessels test. Blood tests indicated that high levels of epicatechin were in their arteries. Apparently the University of California study was the longest clinical trial to date using dark chocolate.

Another study done in Germany indicated that consuming dark chocolate can help lower blood pressure. One group was given white chocolate and one group dark chocolate. The test subjects who ate dark chocolate had a significant drop in blood pressure (by an average of 5 points for systolic and an average of 2 points for diastolic blood pressure). Those who ate white chocolate did not.

The only sad part of this story is that more is not better. You can’t eat all the dark chocolate you want. The people in the study ate about 3 ounces a day. Also try to buy dark chocolate bars that contain at least 70 percent cocoa content.

The Aiken’s Public Service Learning High School program offers students the chance to earn college level credit for the work the students do. Students will have the chance to work with professionals in law enforcement, firefighters, and the emergency medical field. Aiken’s Public Service Learning High School will also have an intensive college preparatory program called University High School.

GlobalHCF and Medical Tourism: A Cure for the High Price of Medical and Dental Costs.

Did you know that over 47 million people have no medical insurance in the United States? Yes, 47 million people! The entire United States population is estimated at close to 300 million people. To give you a perspective of that number – it is more than the estimated aggregate populations of Tennessee, Georgia, Alabama, Florida, Mississippi and South Carolina. The number of people without dental insurance is estimated at 120 million. And, we have not started to talk about the under insured of our population. But I suppose these numbers will do. They are so large that it’s hard enough for most of us to grasp anyway.

When we talk about the uninsured many think that we are talking about the poorest of poor in our country. And while that is partially true, it does not make up the majority. Approximately 80% of the United States is made up of small to medium size enterprises or businesses – called SME’s. Many of these SME’s can not afford the cost of providing insurance coverage to their employees any more than the employees can cover the cost of premiums on their own. Many of these uninsured and underinsured are the working population, the retired, the elderly, those on fixed incomes, single parents, and yes, the working poor. But these folks are not the only people who can not afford medical and dental insurance. The owners of the SME’s can not afford insurance for their own families as they struggle to compete in today’s increasingly competitive global market. Even people who have jobs that help cover some of the cost of insurance premiums find they are being asked to assume more and more of the cost yearly. Even big business is struggling under the cost of health insurance in the United States.

Increasing medical costs and decreasing health benefits, is taking a heavy toll on those with either no insurance or a limited coverage. At an age when health benefits are most sought, people struggle to stay healthy instead, for fear of rising medical bills. But an accident or a major illness can completely wipe out a lifetime of savings and quickly put either a person or a business into bankruptcy. We also increasingly see in the news of large health insurance providers systematically dropping those who have paid their premiums religiously just when they need it the most. For a business it’s difficult to hire and then keep good employees when they are seeking medical and dental benefits for their family.

I guess all of us can see why this problem has been given so much coverage in politics today. And while it is a worthy banner to fly at election time – it is one illness that is going to be very hard to cure. And if there is a cure it will be slow arriving.

One of those SME’s of the Upper Cumberland’s is addressing this issue of health care for businesses and individuals alike. He can tell you of procedures ranging from drug addiction assistance to heart surgery where in every case the cost was a fraction of what it would cost in the Unites States.

“Everyone read the front page of the Tennessean recently which discussed this current trend in businesses and individuals alike” he said. In fact search of the internet also shows several very positive documentaries from CBS, FOX, NBC and 20/20 concerning this practice. In fact this author had difficulty finding someone who had a negative experience with this process. Dr Thomas attributes that to the “increased standards of care a person receives at these hospitals”. Almost all of the hospitals literally cater to the foreign visitor like a dignitary. They are offered the very best care as they know their reputation depends on what is brought back to the U.S. GlobalHCF follows and tracks every customer and patient with the hospitals, Complaints of any kind must be answered and a plan put in place to prevent it from happening again. Dr. Thomas says the worst complaint is timing. So many people are taking vacations and getting treatment in the medical tourism that it’s difficult to move fast enough. “Your health is a matter we take seriously” he says. It is very important that the client talk with the doctor about their procedure, that the doctor reviews the files of the patient and that the patient checks out the hospital and physician thoroughly. “We don’t like surprises and insist on everything be perfect before you arrive” Dr. Thomas remarked.
